Attention All Pre-K & Grade 6 Families

The State of CT requires all students entering Pre-K3, Pre-K4, Kindergarten, and 7th Grade to have an updated Health Assessment form, detailing a recent health physical and signed by the child’s pediatrician, on file with the school nurse prior to the start of school in the Fall.  If your child has their annual physical in the months ahead, please be sure to request a copy of this form or print a copy below and bring it with you to the appointment.

Entering Kindergarten or Grade 7 in Fall 2022: State of CT Health Assessment Form – Kindergarten (aka Blue Physical Form)

Entering Pre-K3 or Pre-K4 in Fall 2022:  State of CT Early Childhood Health Assessment Form Pre-Kindergarten (aka Yellow Physical Form)
